Salt Lake City, Utah | Full-Time

We are currently hiring a full-time Stratford Case Manager to join our multidisciplinary housing and behavioral health team in Salt Lake City.

About the Role:

The Stratford Case Manager provides comprehensive case management and supportive services to residents living in First Step House permanent supportive housing program. This position works closely with individuals experiencing serious mental illness, substance use disorders, and co-occurring conditions to support housing stability, recovery, and community integration.

This role involves:

  • Building supportive relationships with residents
  • Assisting clients in identifying goals and service needs
  • Coordinating care and community resources
  • Supporting engagement in treatment and recovery services
  • Facilitating life skills and supportive groups
  • Providing transportation and appointment coordination
  • Collaborating with multidisciplinary clinical and housing teams
  • Services are provided both onsite and in residents' homes using trauma-informed, recovery-oriented, and strengths-based approaches.
  • This is a meaningful opportunity to make a direct impact by helping individuals build stability, independence, and long-term recovery.

Salary:

  • $46,353.60 annually

Qualifications:

  • A Bachelor's degree and an active license in the State of Utah as a Social Service Worker (SSW) is strongly preferred.
  • Case management experience required
  • Experience with individuals experiencing mental health concerns and/or substance use disorder is preferred

Key Responsibilities:

  • Provide ongoing case management and service coordination
  • Conduct regular resident meetings and home visits
  • Support housing recertification and benefit eligibility processes
  • Connect residents with employment, education, healthcare, and community resources
  • Maintain accurate and timely documentation within HMIS and KIPU
  • Facilitate life skills and psychoeducational groups
  • Participate in multidisciplinary treatment planning
  • Provide client advocacy and supportive engagement
  • Assist with transportation and community integration activities
