ACT Case Manager

We are seeking a compassionate, resourceful, and community-focused Case Manager to join our Assertive Community Treatment (ACT) Team. This role is ideal for someone who is passionate about helping individuals overcome barriers, access resources, and build stable, meaningful lives in the community.
As an ACT Team Case Manager, you won't be sitting behind a desk all day. You'll work directly with clients in their homes, shelters, community settings, and wherever support is needed. You'll serve as an advocate, coach, and connector, helping individuals navigate complex systems while empowering them to achieve their own recovery goals.

About the ACT Team
The Assertive Community Treatment (ACT) Team provides intensive, community-based services to adults living with serious mental illness, including:
Our multidisciplinary team includes psychiatrists, nurses, therapists, case managers, peer support specialists, and substance use counselors working together to provide comprehensive, person-centered care.
